IN THE MATTER OF THE CANCELLATION OF MOTOR

CARRIER CERTIFICATE NO. 5634

ISSUED TO THE PAUL GREGORY FORWARD

ON AUGUST 23RD, 1988

 

 

 

 

WHEREAS  the Board of Commissioners of Public Utilities having satisfied itself that Paul Gregory Forward, holder of Motor Carrier Certificate No. 5634 issued by the Board authorizing the services attached thereto has by letter, dated September 28, 2000, requested cancellation of Motor Carrier Certificate No. 5634.       


 

IT IS THEREFORE ORDERED THAT:

 

Motor Carrier Certificate No. 5634 issued to Paul Gregory Forward be and is hereby cancelled with effect from June 6, 2001, and said Motor Carrier Certificate No. 5634 together with all Schedules attached thereto be returned to the Board.
